PLACE Application for Participating Agencies
Minimum Requirements for PLACE Participating Agencies:
1. Agency must designate one point-of-contact to oversee PLACE participation for agency
(including all programs/projects that may participate).
2. Agency must send a representative to required PLACE meetings and trainings.
3. Agency must ensure clients referred to Close to Home meet these basic PLACE eligibility
criteria before referring:
a. Experiencing homelessness per one of HUD’s four homeless definitions AND
b. Client must not be a registered sex offender (RSO) AND
c. Ensure the client has a demographic profile and HUD UDEs created in HMIS AND
d. Have 1+ significant rental barrier (must document upon referral)
4. Agency must commit to providing the following support to PLACE clients:
a. Serve as a point-of-contact for the client for the duration of the (1) year lease
contract between household, property manager/owner, and Close to Home.
b. Work with eligible households on an individualized goal plan to address issues that
led to their barriers to accessing and retaining housing including training on being a
responsible tenant, money management, or potential legal issues as needed.
c. Review lease agreements with clients to ensure they fully understand tenant
responsibilities.
5. Agency must complete a PLACE Client Enrollment Form and Authorization Form to
Close to Home and receive Approval for each individual household for the PLACE program.
6. Using a client's choice approach, help households identify which properties they want to
apply to for housing.
7. Submit a signed lease/rental agreement and Move-In Condition Report to Close to Home within
14 days of client rental move-in.
8. Upon Close to Homes approval and the client’s enrollment into the PLACE program, the agency
should monitor the client’s status in housing. If there is damage to the unit or the unit
is abandoned, Agency must submit a Request for Funds form to Close to Home along with any
additional documentation.
9. If Agency wishes to extend client’s enrollment in PLACE beyond the first year, agency
must submit a new Client Enrollment Form and Authorization form to Close to Home for
approval 60 days prior to the end of the lease. Close to Home will notify Agency is extension is
possible based on availability of funds.
10. PLACE Participating Agencies that they are responsible for data quality corrections of
UDEs resulting from HMIS monitoring.
As the administrator of the PLACE Program, Close to Home will:
1. Close to Home will complete the project enrollment for PLACE program participants in the local
Homeless Management Information System (HMIS). PLACE Participating Agencies will
be responsible for creating the referred client’s demographic profile and for entering the
HUD Universal Data Elements in HMIS before submitting the client for approval to
Close to Home.
2. Participate in HMIS and enter all PLACE households into the local HMIS system
(*domestic violence agencies funded by VOCA should consult with Close to Home as special
rules apply).
3. Respond to new Participating Agency applications or Client enrollments within 3
business days.
4. Review documentation provided by both agency and property manager/owners to
ensure fair assessment of property damages.
5. Process claims for eligible property damages up to $500 (minus security deposit).
6. Evaluate and approve claims for property damages above $500.00 but not exceeding
$1,500.00 (proof of damages required)
7. Ensure funds (once approved) are disbursed to property managers/owners within 3-5
business days.
