This is an anonymous bullying, drug, and/or weapon report form.  If you have concerns that a child is being neglected or abused, please call Carroll County Department of Social Services at 730-3130 between 8:00 am and 5:00 pm, or call 1-800-552-7096 after 5 pm. You can request to make an anonymous report.

The information from this form will be submitted to the principal by anonymous email.  Please read the questions and provide as much detail as possible.  Thank you.

"Bullying" means any aggressive and unwanted behavior that is intended to harm, intimidate, or humiliate the victim; involves a real or perceived power imbalance between the aggressor or aggressors and victim; and is repeated over time or causes severe emotional trauma.
