The Department of Early Learning and Care requires Quality Care Connections to survey in-home child care programs about the number of vacancies in your program on a quarterly basis. We use this information to update your program profile in Find Child Care Oregon, the parent referral database. Please fill out and submit the survey as soon as you receive it. If we do not receive your survey, you may be contacted by phone for an update. If you have several sites that share one email address, please submit a separate form for each site. Please help us meet this Department of Early Learning and Care requirement by completing the survey accurately and in a timely manner. Thank you for your time and support.
