2025 CCCHS: Rose Cologne RSVP & Volunteer Nomination 
The Rose Cologne Volunteer Recognition Dinner will be held on Thursday, April 24, 2025 at the Mountain View Country Club.

Time: 5:45 PM (Cash Bar) with Buffet Dinner to be served at 6:39PM PM.

Please complete the following RSVP for your volunteer and all attendees from your agency. CCCHS will host (1) volunteer ticket for dinner; additional tickets are $50/person (table of 10 receive a discounted price of $450, includes volunteer). DEADLINE TO REGISTER IS FRIDAY, MARCH 28.
