Lockers will not be assigned until all required forms are turned into the main office. This includes the following items: Emergency Card, Attendance Policy, as well as the Publicity Form and Locker Letter Tear-Off. In addition, student's need to have submitted a More Than A Meal (School Lunch) Application (https://www.myschoolapps.com/
) and their parents need to have created an LAUSD Parent Portal Account (Find Help Here: https://achieve.lausd.net/Page/10470
There should be no need for students to share a locker, but if you plan to do so, please provide the information for BOTH students here on the form. Students are responsible for any items kept in the locker.
You will choose THREE locker numbers that you are interested in and list them here. According to grade level, you must choose from the following ROWS of lockers: Freshman=FOURTH/BOTTOM ROW, Sophomores=THIRD ROW, Juniors=2ND ROW, Seniors=1st/TOP ROW. If you would like to request a locker that is not in your designated grade level row AND YOU HAVE A VALID REASON (medical, height, etc.) please provide an explanation in the last question.
