Photo Permission and General Release of Liability: I hereby authorize WAI and its affiliates to publish any photographs or videos taken of me or my minor child(ren) for use in WAI’s printed publications, website, and other mediums, and for training purposes. I release WAI and its affiliates from any expectation of confidentiality and attest by my signature that I am the parent or legal guardian of the child(ren) listed and that I have authority to permit WAI to use said photographs and/or video for promotional and training purposes. I acknowledge that participation in the event is voluntary.
