Face Masks for Everyone
In the midst of the COVID-19 pandemic, Root Access is organizing an effort to make face masks for the community. If you need a face mask, please fill out this form and we will be in touch.
For more information, see our website:
How many masks are you requesting?
We can't make any promises, but we will do our best.
Is your request on behalf of a business, organization, or community effort?
No, this is a personal request for me and/or loved ones
If yes, what is the name of the business, organization, or community effort?
Are any of these masks for anyone who is considered at-risk? If so, please explain.
We will try to fulfill all mask requests, but we're prioritizing masks for people who are at risk -- elderly, immunocompromised, healthcare workers, community volunteer efforts, and those who must interact with the public on a daily basis.
Can you make a donation?
We're asking for a $5 donation per face mask, which goes to help cover the cost of materials. If you're unable to make a donation, don't worry about it! We're just trying to get as many masks into the community as possible.
Yes, I'll make a donation when I receive my masks.
Keep your mask clean!
Masks should be washed regularly -- we recommend at least once a day and after every potential exposure or public outing. These masks can be washed and dried in a regular washing machine and dryer on high heat.
Yes, I understand.
Assumption of Risk
While cloth masks are better than no mask, they are made with cotton and flannel fabric, and are not a 100% guarantee against the spread of COVID-19.
Yes, I understand the risks.
