Conway Area Humane Society DOG Adoption Survey
We are open for adoptions from 12-4pm Thursdays through Mondays. All adoptions require a pre-approved survey to be on file.

You must be 21 with a valid identification that matches the address listed on this survey.

Adoption approval can take 24-48 hours. Approval notification may be done by email or telephone (please check your spam/junk folder or call the shelter if you do not hear back in 48 hours!)

All animals can be placed on hold for a $50 fee if you are approved to adopt before they are ready to leave.
Please note, the hold fee does not count toward the adoption fee. Adoptions staff have the final say on all adoptions based on appropriateness of an animal for the experience level and environment you can provide. Not all dogs are appropriate for all people, our main goal is making sure an adoption is successful.

Approved applications are kept on file for 3 months.

Crate training *
A new dog should never be left to have free reign of a home when you cannot supervise them. Crates are an excellent tool to keep your new dog and your home safe when you cannot be home until you know they can be trusted alone. When done correctly and not abused (used for too many hours a day), your dog will love their crate and feel safe while in it.

Please note: We rarely if ever guarantee a dog that has been living in a shelter kennel will be house trained once in a new home. This is training that is necessary for all dogs leaving the shelter, please be ready and willing to put in the time necessary to house-train your new dog.
I understand that many puppies are born with intestinal parasites that have been passed down by their mother and my puppy will likely require a follow up vet visit to address this. *
Adopting a puppy requires follow-up vet care to ensure they stay healthy. All puppies receive a fecal test and dewormer prior to adoption, but it is recommended by some veterinarians that puppies be on a regular monthly deworming scheduled until 6-8 months of age.
