Honors and Awards Committee Initial Consideration for Nomination
This form will be used to do a pre-screen of all Candidates to ensure that they meet the minimum qualifications and are eligible for the specific award. Once eligibility has been determined, the Nominator will be notified. Additionally, the Candidate will be contacted to move forward with completing and submitting a full application packet to the HATA Honors and Awards Committee.

Ka ʻOi Kelakela (Most Distinguished) Athletic Trainer Award
For outstanding contribution to the profession of Athletic Training and their place of employment.
Minimum 20 years as an active certified member.
Ke Alakaʻi Nui (Excellence in Leadership) Athletic Trainer Award
For demonstrating leadership in Athletic Training at either the state, district or national level.
Minimum 15 years as an active certified member.
Ke Kākoʻo Nui (Service) Award

For involvement in professional associations, community organizations, grassroots public relations efforts and service as a volunteer athletic trainer.
No minimum of active certified membership
Ka Mea Kūhaʻo (Special Recognition) Award
Examples: Lifesaving or heroic deed performed by an AT. Does not need to be an AT, you can nominate an Athletic Director, Physician, Physical Therapist, Legislator, etc... who has supported or enhanced the profession of Athletic Training.
No minimum of active certified membership
