2022-2023 Boyd Orchestra Required Forms
We are so excited to be starting a new school year in McKinney Boyd Orchestra! In an effort to be efficient and organized, required Orchestra-specific signatures/information have been consolidated to this Google Form. For those students interested in enrolling in private lessons, a separate link will be provided. Please review the full documents (available in links throughout this form), and complete this form together with your child for each student in orchestra. If you would like a paper copy of these forms, please contact an orchestra director, and we can arrange to get them to you. Thank you in advance for your participation in the Boyd High School Orchestra program!

Additionally, all students enrolled in orchestra are REQUIRED to complete three District-specific forms in order to participate each year. If you have already completed these forms for athletics, you must complete them again for orchestra. These include the Co-Curricular/Extra Curricular Contract, Random Student Drug Testing, and MISD Emergency Contact Form. These forms may all be filled out electronically and can be found via the following link: Required District Forms
