NDLA Challenge Incident Reporting Form
Please fill out this report as soon as possible after each challenge to confidentiality of library records or library material. This will enable the NDLA Intellectual Freedom Committee (IFC) to offer assistance, should you desire it. In addition, the information you provide allows the IFC to spot any systematic attempts to challenge confidentiality of records across the state.

Challenges are reported to NDLA membership in the IFC's annual report. The report does not identify specific incidents or libraries. Data about challenges is occasionally requested by outside researchers (e.g., academics studying censorship). If you do not want any identifying information to be shared with these members of the public, you may indicate at the end of the survey that you want your response to remain completely private.
