Awards will be announced by November 20, 2018.
• Each Application requires the following documentation to be uploaded through this Google Form: I. A copy of your organization’s most recent audited finances
• The following supplemental documentation is optional to upload: I. Letters of support from community partners II. Copies of existing outreach materials, organization chart, etc III. Photos, maps, or other visual aids
• Applicants who are awarded a contract will need to submit the following documentation: I. Proof of insurance II. A copy of your organization’s 510C3 IRS Determination Letter (NOTE: Organizations without 501c3 Certification may partner with a fiduciary to carry out a Seed Grant Project.)
Incomplete, invalid or late applications will NOT be considered.
