Transcript Request Form 2018-2019
Submit this form for EACH transcript request. Complete only the questions that pertain to your request. Questions are to be completed in entirety to ensure that transcripts will be sent properly to the correct address.

Please submit request at least 2 weeks before postmark deadline or ONE MONTH before received deadline. Mail delivery can take a week or longer in some cases, so requests in advance are encouraged. Transcripts are sent out weekly on Wednesdays.

Note: Last minute requests may not be received by the deadline, so please plan accordingly.
