State Farm agents who write UAS policies.
Many SF agents are unaware of the fact that they can cover drones under the personal articles policy. Some will cover it, but with a high deductible or other "catches". This form will allow others to find agents that cover UAS or drones without needing to call multiple agents to find the best deal.
