Before you begin exercising your instructor needs to know some information about you to ensure the exercises are safe and effective. If you are between 15-69 years of age, this PAR Q checklist will assist you in making a decision about your state of health before you change or increase your physical activity behaviour. Common sense is your best guide for answering these questions.
This Par-Q is valid for those aged 15-69 years old. If you are over 69 years of age, it is advisable to check with your doctor before you begin and provide reasonable documentation that it is safe to commence activity.
You should complete the PAR Q form every 6-12 months whilst continuing to exercise with us, and inform us of any changes to the answers on this form, as soon as possible. All sections must be filled out in order for you to be allowed to participate in classes/training. Any errors/ missed tickboxes on the form will invalidate the form and you may need to complete it again.
Upon completion of the form, you will be sent a digital copy for your records. Thank-you for taking the time to ensure your safety during exercise.
