Team Official Appointment Form

Please note these guidelines when submitting.

3.3.16 Each team shall have at least one team official that is the same gender as the players on the team.

3.3.17 A maximum of four (4) Team Officials can be carded per Team: 1 Head Coach, 1 or 2 Assistant Coaches, 1 Team Manager. All team officials require Club approval prior to being registered/carded to the team. The Request to Appoint Team Staff form must be completed and submitted to the Development/Competitive Coordinator.

3.3.18 Registered Team Officials are expected to maintain regular attendance and participation in team training, games, tournaments, and other team-related activities.
