Please record the address or most accurate description of the locale of the sighting based on what you recall. *

Was the deer alone or in a group? If the deer was in a group, to the best of your ability please record the number of individuals and the composition of the group, for instance "3 deer total: 2 does and one fawn". We welcome any information you can provide. *

Did any of the deer have ear tags? If so, what color were the tags and, if visible, what were the numbers? *
