This form is used to better understand what type of training your daughter is seeking. After reviewing the information you provide, Carrie will be in touch to firm up the details and work out training times. Please note that we are only able to accommodate a certain number of players wanting private/buddy sessions. If we are at capacity, this form will serve as a waitlist.
