WMSC Facility Booking Request Form
This form will be used for all field change requests, additional field time requests, practice cancellations, meeting room bookings. The purpose of this change to procedure is to ensure more consistent and accurate communication between teams and the club. An hopefully reduce the number of emails in all of our inboxes. :)

1. All inquiries will be acknowledged and forwarded to the city within 2 business days
2. Each requested change will require a separate form submission (.i.e. booking "2 practices at Northfield Tuesday and Thursday @ 6:30-8pm" will require 2 submissions)

Field Cancellation Policy
<30 days prior to practice, team will not be charged for the field time.
> 30 days prior to practice, team will be invoiced unless another team uses the practice time.
>10 days or less prior to practice, team is invoiced for the time.
