Welcome to Mendez Middle School!
There are 3 steps to complete your student's enrollment:

1) Fill out this form
2) Send a copy of student's birth certificate and shot records to the school
3) Complete the enrollment packet found online and send it to the school

All documents and records can be sent securely to mendez@thirdfuture.org.
