Delta English School (DES) promotes and maintains a safe, educational environment for all Titans. DES closely monitors health guidance on SARS-CoV-2 (the novel coronavirus) by National Emergency Crisis and Disaster Management Authority (NCEMA), Ministry of Health and Prevention (MoHAP), Sharjah Private Education Authority (SPEA), World Health Organization (WHO), and Centers for Disease Control and Prevention (CDC).

This COVID-19 Reporting Form will alert Osler Health Center when you test positive for COVID-19. To comply with Sharjah public health requirements, DES is required to share all positive case information with SPEA.

Submit the COVID-19 Reporting Form only if you receive a latest confirmed positive COVID-19 test result. DO NOT submit this Form if you: (a) have a negative or pending COVID-19 test result; (b) experience new or worsening COVID-19 associated symptoms; or (c) believe to have been exposed to someone with COVID-19. Submit a new COVID-19 Reporting Form if you become reinfected and test positive for COVID-19. If a student test positive for COVID-19, the parent or legal guardian must complete and submit this Form. All information submitted on this Form is secure and confidentially released to authorized entities. You may be contacted by DES for further information.
