Please check off in the boxes for each of the below terms and conditions. The Syllabus Contract must be completed, signed digitally by both student and parent/guardian, and submitted by the deadline for the Syllabus Contract pass/fail grade.
We have read the class Syllabus in its entirety and agree to all the policies and procedures therein.

We understand that an instrument is needed for this class and that it is our family responsibility to acquire an instrument and pay any fees affiliated with the instrument to the store or school.
We understand that all band students must attend any mandatory after school rehearsals and performances.
We understand that a concert uniform is required for performances and that all band students must have the correct concert uniform prepared in time for performances according to the uniform requirements mentioned in the Syllabus.
We understand that is it the responsibility of both the child and parent/guardian to keep track of grades and progress on PowerSchool and will email the teacher for gradebook concerns.
We understand that is it required for all band students to have a Remind101 account for this class where notifications are receiving to the student's phone, and that parents/guardians may also join.
