In consideration of my and/or my child's participation in this activity, I hereby release and discharge RISE Water Polo, LLC and it's representatives, successors, and assigns, including the pool facility and their representatives and staff, from any and all liability arising from accident, injury and illness that I or my child may suffer as  a result of my (our) participation in this activity.  I agree and I recognize that water polo involves swimming and involves contact.  I attest that my child can both swim and tread water unassisted, and my child has no medical condition which may limit his/her ability to participate in this camp.  Additionally, I freely accept responsibility for my child's ability and behavior while participating in any RISE Water Polo activity.  I understand that the coaches and leaders of these activities can cancel our participation at their sole discretion, and I will be refunded the fee (pro-rated).  As agent, parent or guardian of the child registered, I indemnify and hold harmless against all claims from any person or representative, the coaches, leaders, parents and other participants of RISE Water Polo.  

I understand that in the event of illness or accident, instructors will attempt to contact me.  However, I authorize any medical care deemed necessary, including life-saving measures.
