*Keep in mind that some committees are larger than others, and that is why there are so many blanks included in this form. You should only include the individuals who will be directly helping you with your event. Many of our volunteers serve on multiple committees, but each volunteer will only receive one 4-day gate pass. If you cross serve i.e. committee chairman and leader, fill out your information in both spots.
Please double-check the emails of your volunteers before submitting. You will not be able to go back and make changes after you submit.
Gate passes will be emailed to the addresses included on this form. The office staff CANNOT submit changes for incorrect emails.
The deadline for submitting this form is Saturday, December 14, 2024. No additions or changes will be allowed after this date.
