Under the Current Public Health Order, any business, entity, large venue, event or public gathering facing unique circumstances not specifically addressed in this Order or seeking to accommodate in excess of the limitations specified may submit a proposed Operational Plan for review and conditional approval by PHHS.

Please note that based on the current transmission level of the COVID-19 virus in Boone County, our recommendation is to host events virtually or postpone to a later date if seeking to exceed the limitations in the current Public Health Order. If still seeking to exceed the current limitations in the Order, an Operational Plan must be submitted and approved.

What are the dimensions/square footage of the event space? What is the fire code capacity of the event space? Please be prepared to submit a diagram of the layout of the event including dimensions of the space/layout. *

How will a list of patrons be collected for contact tracing? Please maintain lists for 30 days. We will only request these lists if there is a positive case related to an event. *
How will you ensure the number at your gathering will not exceed what you are asking for? *
Nature of the activities conducted on the premises (including information about meal service, bounce house, etc.). Will there be entertainment activities such as games, music/live band, etc? *
How will all social distancing requirements be met and enforced? This includes maintaining at least a 6-foot social distance from other individuals, washing hands with soap and water for at least 20 seconds or using hand sanitizer with more than 60% alcohol, covering coughs or sneezes with something other than hands, regularly cleaning high-touch surfaces, and not shaking hands. Please be prepared to submit visual aids or seating diagrams. *
Describe your health screening process (reminding attendees to stay home if sick, fever, symptoms, travel, etc. and checking or symptoms as attendees arrive). *
How will you limit congestion/congregating upon entering and exiting the event, as well as throughout the entire event space (including restrooms)? *
Will food/drinks be consumed at this event? If so, how will they be served? *
Will alcohol be served? *
What is your disinfection plan for common touch points? What EPA approved disinfectant will be used? *

Reminder: Submission of a plan DOES NOT equal approval. Your plan needs to be reviewed by PHHS. We may have follow-up questions. Our team is working diligently to review and respond to all plans.
