CLBC Web Site Survey
August 2017

Community Living BC (CLBC) is building a new web site. This is part of our effort to provide better information and strengthen relationships with those we serve, a key goal in our new strategic plan ( ).

We know Individuals and families may use our web site to find information about CLBC, what services we provide, the process to get services and what they can expect to receive. We want to provide this information in a way that is easy to understand, because we recognize the process can be overwhelming.

As an individual or family member , you can make an important contribution to this project, and our strategic plan goal of improving relationships, by taking a few minutes to fill out this online survey to provide input on:

> How you use the CLBC website

> The topics of information that you need on the CLBC website

> The types/style of information that you like best

Your responses will help us refine and improve our website structure and information, and identify opportunities for new resources. Please be assured that all responses are confidential and anonymous. You are not obligated to participate, this survey is completely voluntary, but we would value your feedback.

We would like your feedback by August 31, 2017.
