Course Reserves request form
Please use this form to request that an item be placed on reserve for your course. You may also use this form to request that an existing course be reactivated online.
Once we receive this form, the requested material will be placed behind the circulation desk. Items on reserve can be borrowed for 4 hours at a time and is for library use only (exceptions may be made for faculty by contacting a faculty librarian). Please refer to the Course Reserves policy at the following URL:
The item title will be added to the corresponding online Course Reserves guide. The Course Reserves guide reflects classes that are being currently taught. Requests for Course Reserves must be made each term.

Reactivate existing Course Reserves for this course (Choose this option for Course Reserves lists that already exist and do not require changes.)
Add a new library resource to the Course Reserves (Select this option to add an existing library holding to a course OR request a new item for purchase.)
Remove a library resource from course reserves (Select this option to remove the item from the reserves shelf and be placed in the regular circulating collection OR to have personal copies returned.)
