This google form should be used by staff/parents/guardians to opt in to the new testing program. In accordance with the Department of Elementary and Secondary Education (DESE) and the Department of Public Health (DPH), our district is updating our COVID-19 Testing Program and will begin phasing in the new changes during the week of January 24th for staff and January 31st for students.   For more complete information about the new testing program please refer to the email sent to WB Families and Staff on January 21, 2022 and information posted on the district website https://www.wbridgewaterschools.org/page/coronavirus-information.A copy of this form will be sent to this email address.
