MPCSD Volunteer and Coach Registration Request
We are grateful for all those interested in volunteering and/or coaching in the Menlo Park City School District.  Please complete this short form to begin the registration and fingerprinting process which is required by state law.  We recommend completing this now as the MPCSD volunteer fingerprinting requirement will be in effect starting November 1st, 2025. 
All school/classroom volunteers, including coaches, must complete the process before working with students/student teams.

If yes to the previous question, when were you fingerprinted for MPCSD?  This is subject to verification and you may need to be reprinted.
You can get fingerprinted at your own convenience. 

Please go to any Live Scan Service Center, such as the UPS Store on Marsh Road, UPS Store on Sharon Park Drive, or Post 'N More on El Camino Real).  This will be at your own cost and pricing varies.

* You will need to bring your MPCSD Live Scan Form and your photo ID when you get fingerprinted.  This form will be emailed to you once you submit this Google Form.  

Where do you plan to complete your fingerprinting?

By state law, volunteers and coaches must provide a Tuberculosis Risk Assessment or a negative TB test dated within the last 4 years.

Your own doctor's form may be submitted, or you can download a TB Risk Assessment Form and have it completed.

Please email your completed TB Risk Assessment OR negative TB test signed by your healthcare professional to HR@MPCSD.ORG .
