For the 2023-2024 school year, the mandatory Test & Fix Water for Kids program will provide free drinking water lead testing to all facilities that regularly serve children in grades 6-8. This includes schools and facilities such as offsite kitchens. Please make a list of all the facilities in your district that regularly serve children in grades 6-8 before filling out this form.

Schools serving children in 5th grade or lower tested for lead in the 2022-2023 school year. If your district has any facilities still needing testing from 2022-2023, please add them in this form.

Please verify that all information in this form is correct to ensure  all of your district's facilities are tested in compliance with the Test and Fix Water for Kids program.

After this form is completed, you will receive an email with instructions to create a sampling plan for each facility so that you can receive a sampling kit. Be on the lookout for emails from our technical assistance team!
