Please use this form to indicate a SAME DAY change for your student's dismissal in an instance where a note could not be sent in. If your dismissal change applies to a day other than today, please send a note in your child's folder on the day in question. In the event of a pickup please ensure your Emergency Contacts are up to date in Skyward noting who is allowed to pick up your student or that the person picking up has a pickup placard. Please call the office if you have any questions.
