2026 Black Creek Adventures Enrollment Form                                        

Welcome to Black Creek Adventures Youth Programs!  We are sponsored by the Fairfield Community Center in East Fairfield.  This year we are offering three options to sign up for on this form.  Please use this form to sign up for our:
 *   after school program at Fairfield Center School
*    February and/or April vacation camps at the Fairfield Community Center
*    summer camp at the Fairfield Community Center

Some information about our programs...

*After School Program*

In the Art Room at Fairfield Center School

Available every day after school, 3:30-5:00

Wednesdays, (Early Dismissal)  2:30-5:00

$20/day, 25% sibling discount - childcare subsidy accepted

*Vacation Camps*

At the Fairfield Community Center

-February 23-27

-April 20-24

Tuesday Field Trip  

Hours of operation:  7:30-4:30

 $185/week - $165/week for siblings.  Childcare subsidy accepted

* Summer Camp *

At the Fairfield Community Center

6 weeks, June 29-August 7

Themed weeks with weekly field trips

Hours of operation:  7:30-4:30

$185/week - $165/week for siblings.  Childcare Subsidy Accepted

All Black Creek Adventures youth programs accept childcare subsidy.  Income guidelines have changed, so more families will qualify!  If you have applied in the past and were denied, please try again!
