In the event that I can not be contacted  I approve this amount to be used to cover medical bills.
As the owner of the Pet/Pets listed above I give Healthypawslv permission to care for the above listed animals. In case of emergency I give permission to Healthypawslv to administer first aid and take the above animals to the vet if needed. I understand that if within 5 days of the bookings end with no proper communication the animal will be considered abandoned, and surrendered to animal control. In case of emergency or loss I will not hold Healthypawslv liable or responsible for lost items or for non negligent injury or death of an animal. I also understand that every day past my scheduled booking I am subject to an extended stay fee of $60 a night per animal. I also understand That if My pet runs out of food it will be an extra $3 charge for in home food per scheduled meal.
