National Public Health Week: SOPHA 5K Registration Form
Thank you for signing up to participate in National Public Health Week's Annual 5K!
This event is hosted by the Society of Public Health Advocates (SOPHA) and will be held on Sunday, April 3rd @ 10am. Participants should show up by 9:30am for check-in. Registration deadline is Friday, 4/1/2022.

**REGISTRATION FEE** READ BELOW**
We will accept non-perishable food items for our Hofstra Dome Satellite Pride Pantry, as our registration fee.
All items must have an expiration date of at least one year from now, and be unused.
We request the following items from our participants, volunteers, and donors:
1. Canned protein
2. Cereal (under 7 grams of sugar, at least 3 grams of fiber)
3. Dried fruit with no added sugar
4. Fruit canned in water or juice (not syrup)
5. Fruit preserves
6. Instant oatmeal
7. Low-sodium soups
8. Nuts
9. Peanut butter
10. Seeds/trail mixes
11. Unsweetened applesauce
12. Whole grain granola bars
13. Gluten Free options
14. Whole Grain Dried Pastas
15. Canned Vegetables
16. Hand sanitizers
17. Essential toiletries
