Please use this form to inform PUP EDI Board of a concern you have regarding your own safety/wellbeing or that of another individual's within the PUP community. You may provide as much or as little detail as you would like. The PUP EDI Board will read and discuss your report, and take appropriate action as YOU and other individuals involved are comfortable. The most important thing to EDI Board is that the responding course of action is taken on the terms of the reporting individual(s). That means we will not take action without discussing a plan with you, if and only if you would like to involve yourself with the report.
With that said, you DO NOT HAVE TO involve yourself with the report. This form is anonymous and does not collect email addresses. If you would like to remain anonymous, that is completely okay.
This form is associated with the google account pupediboard@gmail.com, meaning it is not linked to a particular student's personal gmail, nor is it linked with Princeton University.
Your responses are CONFIDENTIAL and will never be shared with any individuals outside of PUP EDI Board without your consent. Confidentiality will be maintained for all of the future, meaning beyond the tenures of current PUP EDI Board members, and beyond the commencement of any and all individuals involved.
