What is the name and address of the recipient?  *Do not put your name and address.  List all names and addresses.  I do not give students generic letters because I believe a personal touch helps the applicant.  I also do not write letters for LOCAL scholarships.   *

List your awards and honors.  This includes any awards and honors you received outside of school as well.

List your paid work experience.  Include your job title, employer, dates worked, and job responsibilities.                                                
List any unique hobbies or interests.
Please describe anything else you would like a person writing a letter of recommendation to know about you.  You could include personal information like financial need, being the first in your family to attend college, etc.  
A copy of your responses will be emailed to the address you provided.
