Berlin CSD Return to School for student
Schools may allow a student to return to school after day 5 of their isolation period (where day 0 is defined as either date of symptom onset if symptomatic, or date of collection of first positive test, if asymptomatic) if they meet certain criteria.  You will be notified by the district as to whether your student is cleared to return to school after a shortened quarantine period.
This form should only be filled out if the student is a confirmed positive and is on day 5. Please complete this form on Day 5, by 2:30PM.
