Milford Schools Foundation Distinguished Alumni Nomination Form
The Milford Schools Foundation Distinguished Alumni Award began in 2010 to honor graduates of Milford High School who have made a significant impact on their communities and professions and serve as exemplary role models for Milford students.

Nominations for the Distinguished Alumni Award are accepted year-round and will be kept on file and considered each year. Distinguished Alumni will be recognized during the annual Night of the Stars banquet.

Please complete and submit this form by March 1, 2019.


All graduates of Milford High School become eligible for nomination five years after their graduation. Nominees must exhibit one or more of the following characteristics:

- Actively lives out the mission of Milford Schools. Their life is an example of Milford’s mission, which includes educating the whole person and preparing students for success in a global society.
- Contributes extraordinarily to his/her profession and/or community.
- Impacts Milford Schools through direct service to the district or has had an impact on students by leading through example.
