Student will complete and submit this Google Form no later than January 31.
Student will download the fillable pdf form from masc1.org and complete (type) all parts. Please follow all directions.
Students must meet all criteria, print the completed application, and then present to their student council advisor and a principal.
If the principal and advisor find that the student has provided all necessary evidence of meeting the criteria, they will each confirm it with their signatures/initials in the appropriate spaces.
Once application is complete and has all the signatures, student will scan pages 3-5 and handouts (for workshop leader section) and create a pdf. Student will then email this pdf to
masc@masc1.org not later than Jan. 31. It is preferred that all submissions are in one pdf. Most school copiers/printers have the ability to scan and create a pdf.
Student must email a Picture of themselves to
masc@masc1.org no later than January 31. This picture will be used on the screens at State Convention. Please make sure it is a good quality (appropriate) picture - no selfies.
Submit only the requested forms/pages indicated. No other items will be considered during the evaluation process.
Applications missing required information, signatures, or not in compliance with the regulations will not be evaluated by MASC.
