2026-2027 School of Choice Application: K-12
Please read Carman-Ainsworth's School of Choice Guidelines before completing application:
• Applications must be completed and signed by the student’s Parent or Legal Guardian. **Please note that we do not accept Power of Attorney as legal guardianship
• A separate application must be completed for each student.
• Approvals and denials are made on the basis of a two-year, physical-attendance student behavior review. School years or semesters in which the student attended virtually are not included in this two-year review. Applications missing required behavior reports will be considered incomplete and will not be reviewed until all missing documents are provided. If there were no behavior incidents to report, please provide written documentation of this from the prior school(s) on their letterhead. Complete behavior history must be submitted by the application deadline in order to be reviewed.
• Please do not submit any documentation other than behavior and IEPs at this time.
• If the number of applications for admission from nonresident students exceeds the number of available enrollment opportunities, enrollment priority shall be given to a sibling of a nonresident student already admitted under this program. Nonresident students shall be selected for remaining vacancies using a random selection process.
• Accepted School of Choice students are allowed continuous enrollment in Carman-Ainsworth Schools as non-residents through graduation unless the student withdraws from Carman-Ainsworth Schools or is expelled.
• Transportation is NOT provided for Schools of Choice students. Parents must provide their own transportation to and from school.
• Schools of Choice students must be on time for school and picked up no later than 15 minutes after dismissal.
• Accepted School of Choice students must agree to comply with Carman-Ainsworth district policies.
