Legends is a physiotherapist-led functional fitness group class designed for older adults aged 50 and above in Singapore. Classes are limited to 12 participants, guided by a physiotherapist trainer.

👟TRIAL CLASS AT JUST $35!  
You may register your interest by signing up for a trial session. Our team will contact you to arrange a suitable slot. 
Please note: submitting this form indicates interest but does not confirm your trial booking. We will respond to you in 3 working days after you submit the form.

⚠️ PRE-TRIAL INTERVIEW (MANDATORY): 
Once your trial session is confirmed, please arrive 20 minutes before class for a mandatory pre-trial interview with our physiotherapist trainers. This assessment helps us understand your needs and ensure the program is safe and suitable for you. 

Please note that completing the pre-trial interview does not automatically confirm your class placement. If we determine the class may not be suitable, we will discuss the best option for you.

‼️ PUNCTUALITY POLICY: 
Participants who arrive only at the scheduled class start time and miss the pre-trial interview will not be allowed to join the session. In such cases, the trial will need to be rescheduled. Your punctuality and cooperation help us maintain a safe and effective environment for all participants.  

NOTE: You may receive help from a loved one to complete this form, but the participant must participate and provide their own information and signature. Forms signed by someone else will not be accepted.
