This form is for any family that needs to enroll teens in Sacrament Prep for Confirmation, a two-year process that can begin as early as 8th grade.

If your child attends Catholic School then they would still need to register for Sacrament Preparation and participate with their parish (use this form if you are a member of Saint Kateri Parish).
