I hereby authorize the staff at Atascocita Basketball Camp to act for me according to their best judgment in any emergency requiring medical attention and I hereby waive and release the camp from any and all liability for any injuries or illnesses while at camp. By agreeing to this release, I understand this permission signifies that photographic or video recordings of my child may be electronically displayed via the Internet or in the public educational setting. This release applies to photographic, audio or video recordings collected as part of the sessions listed on this document only. *
