This is what we will use on the volunteer scheduling spreadsheet which is visible on the web to those who have the link.  Consider using your Fetlife, Hypbook, MCforums etc. nickname.  If this name is different than the name you want printed on your badge, please clarify with us!

Please put in an e-mail address that you check regularly! Please reply to our e-mails because otherwise we don't know that you're reading them! Most of NEEHU's organization is done using google docs, so we would prefer you used a Google-compatible address.  

By providing your phone number here, you are enabling the volunteer coordinators to call you if needed to facilitate planning or in the event of an emergency.  This is totally optional.  You can also send your # by e-mail to mephki@nehg.info, via fetlife message, or any other way you desire.
