Hello! Thank you so much for showing interest. The information in this survey is being used to support a developing non-profit focused on improving mental health support and access in Memphis. Although the organization is not officially registered, the information here will be used to guide its leading steps. This survey is not affiliated with any corporations. and is not being funded by any entity whatsoever. Your information will not be sold and is anonymous by default. This survey is volunteer-based and not for commercial use. The purpose of this survey is to gather information about mental health issues in Memphis, identify the most common challenges, and learn which demographics are most affected. This information will then be used to guide community outreach. such as Your answers serve as a reference and may be cited or given to the general public. (For example, writing in an online post that 17% of all respondents aged 10-18 suffer from anxiety). Answers will be cited in summary form, so no individual responses will ever be shared.
