I agree to hold the MICS staff and the officers and board members of Mabuhay, Inc. harmless from any liability, financial, or otherwise that may arise from my child/ren ‘s participation in all the Mabuhay, Inc. Culture School’s (MICS) activities and endeavors.

The Mabuhay, Inc. Culture School (MICS) is a project conducted by Mabuhay, Inc. Throughout the school year, (MICS) will conduct activities that may be publicized by Mabuhay Inc. Publicity activities include photographs or videos of individuals or groups of students, or student works (including, but not limited to, classroom activities, artistic works, athletic activities, performances and competitions) that may be used in websites, social media such as Facebook and Twitter, print or online publications, or videos. The information shared under this publicity release is generally not considered harmful or an invasion of privacy if released. Please check ONE of the two statements below. This Publicity Release Form will remain in effect through September 2024.
