The word ‘Transition’ is used to describe the process of moving from children’s services to an adult care team.

This might be something that your healthcare team talk to you about for a long time before it happens but once you reach a point where you are being given specific information, and you and your healthcare team are taking steps to prepare for how and when this will take place, then you are known as ‘in transition’. This might also involve meeting with members of your new adult healthcare team and spending time with other patients in a special ‘Transition Clinic’.

This survey tool has been designed to help us understand how you feel at different points during your own transition process so that we can see how transition is working, and to find ways we can better support patients like you, their families, and health providers.

We’d like you to complete this survey after each appointment during your transition so in order to take part, we’ll ask you to provide your name so that we can keep track of your individual answers. Your name will never be shared with anyone outside Metabolic Support UK and will also be removed once you’ve completed the survey for the last time.
