Summer 2024 Basketball Camp Sign Up
Hi! Please sign your daughter up for the 2024 summer basketball camp. 
  • High School (7-12) will run from 9am -12pm
  • Elementary (K-6) will run from 12:30pm - 2:30pm
The cost will be $35 with a t-shirt included. Please make checks payable to Giltner Public Schools. 

Forms Due by May 8th to ensure we get the shirts before camp. Any form submitted after May 15th may not receive a t-shirt in time for camp.

CAMP INFORMED CONSENT AND WAIVER FORM EVENT WAIVER:

I HEREBY AUTHORIZE THE STAFF OF THE 2024 Giltner Hornet Basketball CAMP TO ACT ACCORDING TO THEIR BEST JUDGEMENT IN ANY EMERGENCY REQUIRING MEDICAL ATTENTION AND I HEREBY WAIVE AND RELEASE THE CAMP FROM ANY AND ALL LIABILITY FOR ANY INJURY OR ILLNESSES INCURRED WHILE AT CAMP. I UNDERSTAND THAT VIOLATION OF CAMP RULES MAY RESULT IN DISMISSAL FROM CAMP WITH ALL TUITION FORFEITED. MY SIGNATURE ALSO INDICATES THAT MEDICAL PERMISSION HAS BEEN SECURED TO PRACTICE AND I HAVE INSURANCE TO COVER ANY INJURIES OR ILLNESS INCURRED DURING CAMP. ALL CAMPERS MUST HAVE HEALTH INSURANCE TO PARTICIPATE IN CAMP.
