Please answer the following questions to register for this year's Homecoming Parade. You will complete this form and submit it now. Then, you will choose one of the ways provided at the bottom of the form to make payment. Form must be completed and payment made by Friday, September 26.

The fee for parade registration is $10.

This fee goes directly to the Student Council group to be used for birthday and congratulations treats for the student body, materials for Stu Co sponsored events like the Homecoming dance, and other operating costs.

You may make checks out to CPHS Student Council.

Payment should be made by Friday, September 26.
