Healthwatch Liverpool is an independent patient organisation. We gather feedback from people who use health and social care services to find out what they think.

We are currently gathering feedback from people who have used the Care at the Chemist service. This is a service where you get medicines for minor ailments for free from a pharmacy. You have been given this survey as you have received this service. We are keen to find out what you think about the service. We would be grateful if you could complete the short survey overleaf.

We will put together all of the feedback we receive and use this to write a report about the service.

The surveys are anonymous and any feedback you give will be in the strictest confidence and will be anonymised before being included in our report. Feel free to be as honest and open as you like with your feedback.

It’s a local rate call but we’re happy to call you back. We are available from 9am to 5pm, Monday to Friday. Outside of normal office hours please leave a message and we will get back to you at a
